Job summary

The Medicine Care Coordinator at Jenner Health Centre will support medicines-related services across Modality Lewisham Division practices, working with the Lead Pharmacist and PCN Manager to achieve KPIs and improve patient care.

The role requires organised, proactive, adaptable individuals who can contribute to innovative workflows and flexible working, with opportunities for career development and NHS pension.

Qualifications

• Organised, proactive and adaptable individuals who are passionate about patient care.
• Ability to work effectively as part of a multidisciplinary healthcare team.
• Experience or interest in medicines-related services is advantageous.

Responsibilities

• Work with the Lead Pharmacist and PCN Manager to support KPIs, targets and performance indicators.
• Contribute to the delivery and coordination of medicines-related services across the Lewisham Division.
• Collaborate as part of a multidisciplinary healthcare team to improve service delivery.
• Support the implementation of innovative solutions and improved working methods.
• Engage with stakeholders to harmonise processes and improve patient care and experience.
