Job summary

Whittington NHS Trust is seeking an enthusiastic Community Health Care Assistant to join a skilled district nursing team. You will provide high-standard nursing care in patients’ homes and contribute to preventing unnecessary hospital admissions.

Job description

The Role:

The post holders will work closely with the district nursing team. You will be expected to provide a high standard of nursing care and contribute to the prevention of unnecessary hospital admissions and facilitate early discharge from hospital. You will work with health professionals providing a high standard of nursing care, which reflects the NVQ3 Care competencies to all patients living in the Community.

Main Duties:

• Participate in the assessment of care needs and the implementation of programmes of care in partnership with patients/carers in their own homes.
• Identify changes in patients’ conditions and, within their capabilities, recognise where significant changes in the care plan are required and report these back to the senior nurse on duty in a prompt and timely manner.
• Liaise effectively with other nursing and multi professional team members within the scope of the Health Care Assistant role and report back to the nurse in charge. Be competent in additional clinical skills and knowledge areas, for example:
• Blood pressure monitoring
• Blood sugar monitoring
• Administration of medicines via gastrostomy plus setting up of gastrostomy feeds
• Administration of oral medication
• Simple sterile dressings
• Best infection control practice Be willing to quickly undertake further training/supervision in other clinical skills, knowledge areas for example:
• Continence re-assessment
• Venepuncture
• Injection technique
• Diabetes care
• Medication review
• Chronic disease management
• Pressure area care
• Bowel care
• Palliative care

Requirements:

• Maths and English level 1
• Completion of Care Certificate
• NVQ Level 3 (Care) or equivalent
• Willing to work towards Level 2 functional skills
• Computer and office skills
• Basic numeracy (drug calculation) skills
• Ability to give appropriate advice to patients and carers
• Ability to use own initiative and at times work unsupervised
• Ability to function well in times of pressure by prioritising and planning workload
• Flexible approach to task and location and working pattern
• Understanding of Primary Health Care
• Ability to work as part of a team
• Meet appropriate competencies in the primary health care / hospital setting
• Excellent communication skills – written and verbal
• Excellent time management skills

Please note that all candidates need to be able to be competent in the following:

• Measuring and accurately recording vital signs, including blood pressure, pulse, temperature, and respirations
• Diabetes care including blood glucose monitoring and administration of insulin
• Administration of oral medication
• Administration of medicines via gastrostomy and setting up of gastrostomy feeds
• Collecting urine samples and undertaking urinalysis
• Catheter care, including changing catheter drainage bags
• Pressure area care and monitoring skin integrity
• Undertaking simple wound dressings and awareness of infection control practice
• Bowel care
• Venepuncture
• Recognising signs of abuse, neglect, or safeguarding concerns
