Placements24 is seeking an experienced Case Manager to support clients in Welkom within the Social Services division. You will assess needs, design care plans, and coordinate access to community resources to improve quality of life and outcomes for individuals and families.

The role emphasizes advocacy, collaboration with agencies, and maintaining confidential records. A relevant degree and proven case management experience are required to deliver client-centered care in a supportive framework.

Qualifications

• A relevant qualification in Social Work, Psychology, or a related field.
• Proven experience as a Case Manager or in a similar social services role.
• Strong understanding of welfare systems in the Free State.
• Excellent assessment, problem-solving, and decision-making skills.
• Exceptional interpersonal, communication, and advocacy abilities.
• Proficiency in client record management systems.

Responsibilities

• Conduct comprehensive needs assessments for clients, identifying social, emotional, and practical support requirements.
• Develop, implement, and monitor individualized case plans in collaboration with clients.
• Coordinate and facilitate access to housing, healthcare, education, and employment services.
• Provide ongoing support, counseling, and advocacy to clients throughout their case management journey.
• Maintain accurate and confidential client records, including progress notes and logs.
• Liaise with external agencies to ensure seamless client support.
