Job summary

Avisena Specialist Hospital is seeking a Pharmacy Assistant to support the pharmacy department in a hospital setting in Shah Alam. You will follow standard procedures and MOH regulations while handling prescriptions, patient counseling, stock management, and daily operations.

Responsibilities

• Ensure neatness and cleanliness of workplace.
• Verify prescription appropriateness.
• Prepare medicines for outpatient and inpatient per doctor prescription.
• Data entry into HIS based on prescriptions.
• Clarify prescriptions with prescriber when in doubt.
• Fill and label medicines as prescribed correctly.
• Prepare extemporaneous preparations as needed.
• Dispense medicines and counsel patients on dosage, frequency and duration.
• Top up medicines for inpatients and manage stock/expiry checks.
• Induct stock from pharmacy daily and report incidents as required.

POSITION SUMMARY:

Pharmacy Assistant assist in the various activities of the pharmacy department. Pharmacy Assistant function in accordance with standard written procedures, guideline and Ministry of Health Rules and Regulations.

DUTIES & RESPONSIBILITIES:

Ensure the neatness and cleanliness of workplace.

Verification of the appropriateness of the prescription received.

Preparation of medicine for the outpatient and inpatient in accordance to the doctor prescription.

Data entry into the HIS based on the prescription received.

Clarify with the prescriber if there were doubt on the prescription.

Fill and label medicine as prescribed correctly.

Preparation of extemporaneous preparation as and when needed.

Dispense medicine, counsel patient (as and when required by informing the patient on the dosage, frequency and duration of the medication.

Top up sufficient medicine for the inpatient.

Indent stock from the pharmacy store daily.

Record of the control medicine dispense to patient as required by the law.

Assist Senior Pharmacy Assistant in managing the stock inventory at the pharmacy to ensure sufficient stock, FEFO, minimize wastage and checking the expiry date.

Able to work extra hours or whenever being called for duty by the in charge.

Comply with the company policy, rules and regulation.

Participate in the staff development program and subject to yearly performance evaluation.

Participate in the company improvement activities.

Interact effectively with other members of the healthcare team.

Maintain the confidentiality of the company and patient.

Take reasonable care at workplace for the safety of oneself, others and to comply with any instruction or measure on occupational safety, health and environment as required.

Report all accidents/ incidents and hazard to the superior and complete the Incident Report Form for each accident / incident.

Perform other related duties as assigned.
