Job summary

Presbyterian Villages of Michigan seeks a Service Coordinator to align programs and services that sustain residents’ quality of life and age in place. You will assess needs, connect residents with resources, and support well-being through community partnerships and care planning.

Responsibilities include psychosocial assessments, referrals, and maintaining files while complying with HIPAA and funder QA requirements. Experience with aging populations is preferred.

Qualifications

Minimum degree in social work, psychology or gerontology preferred.
Two or more years of experience in social service delivery with elderly and/or family population.
Demonstrated working knowledge of supportive services and resources in the area served.
Ability to advocate, organize, problem-solve, and deliver results for residents.
Good communication, comprehension and interpersonal skills.
Working knowledge of Windows, Microsoft Office Suite and resident software data systems (AASC online, Care Guide, RealPage or similar).

Responsibilities

Coordinate programs and services to support resident quality of life and aging in place.
Network with community organizations to offer affordable and new services to residents.
Conduct psychosocial assessments and evaluate resident or family needs.
Maintain resident files and monitor supportive services; provide referrals.
Prepare and submit quarterly/annual reports and ensure HIPAA and QA compliance.
