Job summary

Mosaic Health, LLC. is seeking a Care Coordinator to support care management in an outpatient setting. The role reports to the RN Care Manager and collaborates with PCPs, specialty teams, patients, and caregivers to improve outcomes and reduce avoidable utilization.

Responsibilities include care plan implementation, chart reviews, appointment tracking, and coordinating services across providers. Telephonic outreach and documentation are also essential parts of the role.

Qualifications

Assists in implementing and monitoring individualized patient care plans.
Performs monthly patient chart reviews to identify care gaps and preventive needs.
Monitors follow-up appointments, referrals, diagnostics, and care plan goals.
Coordinates services among PCPs, specialists, hospitals, SNFs, home health agencies, and community resources.
Conducts telephonic outreach to patients and caregivers as directed.

Responsibilities

Assist the RN Care Manager in care plan implementation and monitoring.
Review patient charts monthly to identify gaps and opportunities for intervention.
Track follow-ups, referrals, tests, and goals to ensure progress.
Coordinate care across providers and settings for continuity of care.
Engage patients and families through outbound outreach to support adherence.
