Job summary

Meridian Health Partners is seeking a Care Coordinator to perform administrative and clinical duties under a licensed healthcare provider. This non-exempt role helps facilitate communication with patients and providers and supports quality patient care.

You will review patients, schedule tele-visits, coordinate with care teams, assist with records and enrollment in Chronic Care Management, and educate patients and families while upholding CMS and HIPAA standards.

Qualifications

• Certified Medical Assistant
• High School diploma or GED
• Valid Texas Driver's license
• Must have reliable transportation
• Minimum of two years' experience in health education, patient navigation, and/or health clinic patient assistance

Responsibilities

• Review assigned patients to make timely and appropriate patient care calls.
• Schedule patients for routine tele-visits with providers.
• Communicate with providers and outside specialists involved in patient care.
• Assist in acquiring patient records and creating care plans.
• Share care plans with patients and families per CMS guidelines.
• Provide patient and family education.
• Ensure CMS Chronic Care Management compliance.
• Assist in enrolling new patients to Chronic Care Management.
• Maintain HIPAA privacy and confidentiality.
• Communicate with interdisciplinary teams (social workers, home health) for patient needs.
• Report errors or issues to supervisor promptly.
• Maintain HIPAA privacy and security of medical records.
• Escalate non-routine inquiries to appropriate level for resolution.
• Adhere to immunization/vaccination requirements at client sites.
