Job summary

Patriot-Enterprises-LL in the United States seeks a Systems Navigator for the Exceptional Family Member Program (EFMP). This role coordinates daily services for Families with special needs, including intake interviews, referrals, and data collection to support planning and program improvement.

The Systems Navigator will collaborate with military and civilian providers, maintain records in compliance with DoD regulations, and deliver monthly reports while partnering with Families to build

Qualifications

• Knowledge of human service principles and practices and family needs in case management.
• Knowledge of laws, policies, and practices governing medical, educational, and family support services.
• Ability to provide professional non-clinical case management and connect families to required care services.

Responsibilities

• Provide home visits to support Soldiers and Families as needed.
• Coordinate with government and civilian providers to locate respite care resources.
• Maintain an information catalog of networks and contacts offering EFMP support.
• Coordinate monthly support groups or workshops.
• Present EFMP Systems Navigation briefings to outline available services.
• Provide information and referrals to Families to help them choose services.
• Collaborate with Families to build resilience and self-advocacy.
• Maintain Soldier and Family member records per Army/DoD regulations.
• Facilitate warm hand-offs to program personnel during transitions.
• Collect data for planning and monthly reports to improve the program.
• Implement policies that connect Families to support services.
