Patriot Enterprises LLC seeks an EFMP Systems Navigator to coordinate services for Families with special needs, bridging military and civilian resources. You will conduct intake interviews, assess needs, referrals, and follow-up care, while collaborating with providers to ensure comprehensive support.

Your role includes home visits, maintaining a network catalog, leading monthly workshops, and presenting EFMP briefings to Soldiers and Families.

Qualificações

• Bachelor's degree in social work, psychology, counseling, education, or related field.
• Two years of experience in family-related case management or social work.
• Knowledge of human service principles and laws governing medical and family support services.
• Must be able to travel and be legally authorized to work in the United States.
• Level 1 Life Coach certification.

Responsabilidades

• Provide home visits to support Soldiers and Families as needed.
• Coordinate with government and civilian agencies to identify respite resources.
• Maintain a catalog of networks and contacts for services.
• Coordinate a monthly support group or workshops.
• Present EFMP briefings to Soldiers and Families.
• Manage Soldiers and Family member records per Army/DoD regulations.
• Facilitate warm handoffs to program staff during transitions.
• Collect data for planning and monthly reporting.
• Implement policies linking Families to support services.
