Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility partner in South Sioux City, Nebraska. The role is PRN and focuses on delivering high-quality occupational therapy services to residents.

The OT Assistant will screen residents, implement treatment plans, communicate findings to families and the interdisciplinary team, and maintain compliant documentation. Ongoing licensure maintenance and professional development are required.

Competenze

• Associate's degree in Occupational Therapy required.
• Must have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure.

Mansioni

• Screen residents to determine the best intervention based on scope of practice.
• Perform treatment plans and follow short- and long-term goals as per the plan of care by supervising therapist.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families, and team members.
• Establish individual resident therapy schedules with team cooperation.
• Conduct therapy according to the plan and ensure proper documentation and billing of OT services.
• Perform all duties as assigned to effectively treat patients.
• Comply with CMS documentation standards with daily point-of-care documentation.
