Caring Hand Home Health Solutions, Inc. seeks an Occupational Therapist to help patients regain independence in their own homes, delivering high-quality, patient-centered care.

This per diem role pays $130 per visit and requires an active California OT license, minimum 2 years of OT experience, and the ability to travel to patient homes. A flexible schedule and strong communication skills are valued as part of a supportive team.

Competenze

• Graduate of an occupational therapy curriculum accredited jointly by the AMA and AOTA.
• Registered with the National Registration Examination of the AOTA.
• Active California Occupational Therapy license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification with AHA.
• Valid driver's license and ability to travel to patient homes.
• Strong communication and organizational skills.

Mansioni

• Evaluate patients' functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills affected by illness or injury.
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and promptly report changes in patient condition.
• Coordinate with the broader care team to ensure seamless, patient-centered care.
• Educate family members and caregivers on the patient’s treatment approach.
• Supervise OT Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
• Perform other duties as required for patient services.

• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
