Job summary

Praxis Care is seeking Support Workers to join a dedicated team delivering high-quality, person-centred care to adults and children. The role covers developing independent living skills, assisting with personal care, and supporting individuals to achieve their outcomes in a respectful, dignified environment.

We offer flexible patterns including full-time, part-time and relief, with 39-hour full-time equivalence pro rata to contracted hours.

Qualifications

• Numeracy and literacy skills.
• Effective communication skills.
• Experience working within the health and social care field.

Responsibilities

• Support and empower individuals to develop or improve independent living skills as identified in person-centred plans.
• Work closely with Team Leaders and managers to prepare and implement individual and group support programmes.
• Collaborate with other Support Workers and Team Leaders to promote a safe working and living environment, resolving conflicts with appropriate language and techniques.
• Ensure that medication is stored and administered in line with the Medication Policy where necessary.
• Familiarise yourself with administrative requirements and record information as required by reporting guidelines.
• Be aware of all policies and procedures, including Health & Safety, and participate in related training.
