Praxis Care is seeking Support Workers to provide person-centred assistance to vulnerable adults and children. You will work with teams to implement tailored support plans, promote independence, and help with daily living activities in a respectful environment.

Roles offer flexible patterns (full-time, part-time, relief) and salaries are based on 39 hours, pro-rata to contracted hours. Training and supervision are provided to meet regulatory standards.

Qualifications

• Attention to detail and clear communication are essential.
• Experience with supporting individuals with learning disabilities is beneficial.

Responsibilities

• Support and empower individuals to develop independent living skills.
• Collaborate with team leaders to implement individual and group support plans.
• Maintain a safe environment and follow policies and reporting guidelines.
• Assist with medication storage and administration according to policy where needed.
