Job summary

Crosscare in Dublin, Ireland seeks a Mid Level, Full Time case management practitioner to provide 24-hour residential support for adults experiencing homelessness. The role involves developing comprehensive support plans, updating PASS, and liaising with external services to secure long-term housing.

This is a one-year fixed-term contract with a 38.5-hour week and a 7-day rota. Reporting to the Deputy Project/Project Leader, the ideal candidate will have a BA-level NFQ qualification and at least

• Experience implementing case management systems and delivering one-to-one support.
• Ability to advocate for clients and liaise with external service providers.
• Ability to work within Crosscare policies and DRHE Pathways Model.
