Job summary

Praxis Care Castlebar is seeking a Team Leader for a new service. Lead a team of Support Workers, design individual support plans, and coordinate care to meet physical, emotional, and social needs of the people we support.

Responsibilities include safeguarding, regulatory compliance, staff supervision, and liaising with families and multidisciplinary teams. QQI Level 5 Health Care and 2 years’ experience are essential; driving licence required.

Qualifications

• 2 years’ experience in a health and social care setting.
• QQI Level 5 in Health Care or equivalent.

Responsibilities

• Plan, coordinate, and implement safe transitions into the service and community.
• Liaise with families and multi-disciplinary teams to promote needs.
• Develop and implement individual and group support programmes.
• Coordinate ongoing assessments and reviews using suitable models.
• Support the physical, emotional, and personal needs of individuals while upholding dignity and independence.
• Ensure medication is stored and administered according to policy.
