Praxis Care is seeking a Relief Support Worker for Cornerstones to support individuals in maintaining independence and improving daily living skills. You will work as part of a dedicated team delivering high-quality care with respect and dignity at the core.

You will follow person-centred plans, assist with domestic, social, and personal care, and collaborate with Team Leaders to implement individual and group programmes while adhering to policies and medication procedures where required.

Qualifications

• Numeracy and literacy skills.
• Effective communication skills.
• Experience in health and social care is desirable.

Responsabilités

• Support and empower individuals to develop or improve independent living skills per person-centred plans.
• Collaborate with Team Leaders to implement individual and group support programmes.
• Promote a safe working and living environment and manage conflict using appropriate techniques.
• Ensure medication is stored and administered in line with policy where required.
• Complete administrative tasks and logging per reporting guidelines.
• Be aware of and participate in all service policies and training including Health & Safety.
