Job summary

Dublin Simon Community in Dublin is seeking a Project Worker for our Step-Up, Step-Down unit. You will support service users to complete admissions, develop social care plans, and conduct regular key-working sessions to assist life planning and move-on options.

You will coordinate referrals, manage discharge planning, and help sustain health, wellbeing and housing outcomes through group work and collaboration with nursing staff.

Qualifications

• Degree-level qualification in a relevant discipline.
• Three years working experience in a social care setting.
• Diploma in Addiction Studies.
• Knowledge of homelessness and mental health, addictions, domestic violence, and related issues.

Responsibilities

• Completion of needs assessment and development of social care plans with service users.
• Engage with service users, building professional relationships with dignity and respect.
• Coordinate referrals and plan for discharge with nursing staff.
• Explore accommodation options post-discharge and aftercare services.
• Coordinate health promotion groups and supervise volunteers.
