Job summary
The Rehab Group in Shannon is seeking a Programmes Facilitator to join our community services team on a permanent contract (35 hours per week). You will develop and deliver person-centred programmes, supporting independence and community participation.
Reporting to the Community Services Manager, you will work as a lone worker, travel as required, and maintain records while promoting safety and dignity in all interactions with service users and families.
Qualifications
- Minimum QQI Level 5 in Social Care or equivalent.
- A third level graduate in Health & Social Care or equivalent desirable.
- Experience supporting adults with intellectual disability and mental health issues.
Responsibilities
- Be a key worker and facilitate Person-Centred Planning with service users.
- Develop and deliver person-centred programmes and maintain records.
- Support individuals to access their community and coordinate activities.
- Work independently as a lone worker and liaise with team members and families.
- Comply with Rehab Group policies and promote independence and safety.
