Job summary

Rehab Group in Shannon, Ireland, is seeking a Programme Facilitator to support service users through person-centred planning and activity programming. The role is based at Shannon Resource Centre with a permanent contract for 35 hours per week.

Reporting to the Community Service Manager, the post involves facilitating independent living, personal development, and community activities, with travel required and plus points for lone-working experience and health & social care qualifications.

Qualifications

• QQI Level 5 in Social Care or equivalent.
• A third level graduate in Health & Social Care or equivalent is desirable.
• Experience supporting adults with an intellectual disability and people with mental health issues is essential.
• Programme facilitation skills are an advantage; client handling experience is advantageous.
• Experience of lone working and community support aligned with New Directions is desirable.

Responsibilities

• Be a key worker to service users and support person-centred planning.
• Develop and deliver person-centred programmes and maintain required records.
• Assist service users to access their community and participate in activities.
• Work independently as a lone worker and liaise with the team to ensure high-quality service.
• Adhere to Rehab Group policies and promote independence and community integration.
