Zusammenfassung

The Rehab Group in Sligo is seeking a Programs Facilitator to lead and deliver person-centred programmes for service users. The role involves facilitating independent living, personal development, and community activities within a quality-lifestyle framework.

Reporting to the Community Service Manager, you will work as a key worker, maintain records, and liaise with families and external agencies. A full driving licence and willingness to travel are required.

Qualifikationen

• Minimum QQI Level 5 in Social Care or equivalent.
• A third level graduate in Health & Social Care or equivalent is desirable.
• Experience supporting adults with an intellectual disability and people with mental health issues.

Aufgaben

• Be a key worker to service users and support Person Centred Planning.
• Develop and deliver person-centred programmes and maintain records.
• Support individuals to access the community and facilitate activities.
• Work as a lone worker and liaise with team members to ensure high-quality service.
• Adhere to Rehab Group rules and promote independence and community integration.
