The Rehab Group is seeking a Programmes Facilitator at Shannon Resource Centre. This role involves facilitating service users in their choices, developing person-centred programmes, and enabling independent living and community participation. A QQI Level 5 in Social Care is required, with third-level health and social care credentials desirable.

Driving is required, and travel is involved. We offer a range of benefits including a company pension, educational leave, and generous annual leave.

Qualificações

• Minimum QQI Level 5 in Social Care or equivalent.
• A third level graduate in Health & Social Care or equivalent is desirable.
• Experience supporting adults with an intellectual disability and people enduring mental health issues.
• Programme facilitation skills an advantage. Client handling experience an advantage.
• Experience of lone working and supporting people in the community in line with New Directions.

Responsabilidades

• Be a key worker to service users and facilitate the Person-Centred Planning process.
• Develop and deliver person-centred programmes for service users.
• Support individuals and groups to access community activities and meet identified needs.
• Work on own initiative as a lone worker and liaise with team members to ensure high-quality service.
• Comply with Rehab Group policies, procedures and health & safety regulations.
• Promote independence and community integration for service users.
• Ensure safe transportation of clients and maintain records.
