Job summary

The Rehab Group is seeking a Programme Facilitator in Stillorgan, Co Dublin. You will report to the Community Service Manager and support service users through person centred planning, developing independent living, personal development and leisure programmes.

Two permanent full-time posts, 39 hours per week, with a starting salary in the listed scale and progression. An equal opportunity employer offering extensive benefits and development opportunities.

Qualifications

• Minimum QQI Level 5 in Social Care or equivalent.
• Health & Social Care graduates are desirable.
• Experience supporting adults with intellectual disability and mental health issues.

Responsibilities

• Be a key worker to service users to facilitate Person Centred Planning.
• Develop and deliver person centred programmes and maintain records.
• Support individuals in accessing community and facilitating activities and programmes.
