Job summary

The Rehab Group in Ireland is seeking a Programmes Facilitator at our Longford Autism Day Service on a permanent, part-time contract (26 hours/week). You will enable service users to choose and participate in person-centred programmes, including independent living, development and community activities, within a quality-of-life framework.

Applicants should have QQI Level 5 in Social Care or equivalent (third-level qualification desirable).

Qualifications

• QQI Level 5 in Social Care or equivalent.
• A third level graduate in Health & Social Care or equivalent is desirable.
• Experience supporting adults with an intellectual disability and people enduring mental health issues.
• Programme facilitation skills an advantage; client handling experience an advantage.
• Experience of lone working and supporting people in the community in line with New Directions.

Responsibilities

• Be a key worker to service users to facilitate Person-Centred Planning and identify required goals and supports.
• Develop and deliver person centred programmes and maintain required records.
• Support individuals and groups in accessing the community by facilitating activities and programmes.
• Work on own initiative as a lone worker and liaise with team members to ensure high-quality service; communicate with external agencies and families.
• Observe Rehab Group rules and promote independence and community integration.
