Job summary

Dovida Ireland is seeking a Care Manager Lead to oversee home care operations and lead a sizeable care team across our Ireland network. You will develop care plans, perform risk assessments, and support the care team while ensuring QA and compliance in client homes.

The role focuses on mentoring caregivers, maintaining strong client relationships, and collaborating with health professionals to deliver high-quality care.

Qualifications

  • Must hold Nursing qualification or Social/Healthcare degree.
  • 4/5 years’ experience in home care or related area.
  • Ability to lead and manage a team.
  • Computer literacy is essential in daily duties.
  • Knowledge of QA, care plans, and compliance is advantageous.

Responsibilities

  • Conduct consultations and prepare care plans.
  • Perform risk assessments in clients' homes and mitigate risks.
  • Lead weekly teams meetings and guide the Care Team.
  • Monitor client and caregiver concerns and resolve issues.
  • Carry out QA visits and review care plans.
  • Ensure care aligns with standards and liaise with family and healthcare professionals.
  • Mentor a large team of caregivers.
  • Develop client relationships and liaise with health professionals.
  • Report to senior management on clinical issues.
  • Participate in clinical audits as identified.

Who we`re looking for

Dovida Ireland is looking for an ambitious “Care Manager Lead” with following qualification and skills

Education/Experience Requirements
  • Nursing Qualification and/or Social, Healthcare degree preferred.
  • 4/5 years’ experience in home care, health care or senior-related industry required, an equivalent combination of education and work experience may be considered.
  • Ability to lead and manage a team.
  • Home Care Sector business acumen skill
  • Computer literacy skills is a must.
The Care Manager Lead will be responsible to
  • Conduct consultations and prepare care plans.
  • Conduct risk assessments in the home and mitigate any risks
  • Conduct weekly teams meeting and support the Care Team with directions & guidance
  • Monitor, mediate and manage both client and Caregiver concerns and issues.
  • Carrying out Quality Assurance (QA) visits in client homes, developing and reviewing care plans for clients and conducting service enquiries and care consultations as needed.
  • Through the QA visits, ensure client care plan is in line with their care needs, following up with family and healthcare professionals as appropriate.
  • Ensuring the care being delivered in client’s homes is meeting HCCI, ISO & Franchise Standards, local policies and the requirements of the Health Service Executive (HSE)
  • Caregiver Supervisions to be kept up to date.
  • Mentoring and supporting a large team of Caregivers to carry out their roles effectively.
  • Develop and maintain client relationships and client family/ caregiver relationships. - Build networks and enhance existing relationships with health care professionals and multi-disciplinary teams in hospitals and various community settings
  • Report and consult with senior management on clinical issues as appropriate.
  • To participate in clinical audit as identified by the organization.
  • Assist with other duties as delegated by the Management Team.
