Job summary

Dovida Ireland is seeking a Care Manager to lead client care planning, risk assessments, and QA visits across our Ireland network. You will oversee care plan development, monitor caregiver performance, and ensure compliance with national standards and healthcare regulations.

You will mentor caregivers, build relationships with families and health professionals, and report findings to senior management. A QQI Level 5 in social/healthcare and 2–3 years in home care are required.

Qualifications

• QQI Level 5 Social/Healthcare required.
• 2-3 years’ experience in home care or senior-related industry required.
• Computer literacy is a must.
• Full clean driver’s licence and access to your own car required.

Responsibilities

• Conduct consultations and prepare care plans.
• Conduct risk assessments in the home and mitigate risks.
• Monitor and manage client and caregiver concerns and issues.
• Carry out QA visits in client homes and review care plans as needed.
• Ensure care aligns with HCCI, ISO & Franchise standards and HSE requirements.
• Mentor and support a large team of caregivers.
• Develop and maintain client and family relationships with health professionals and hospitals.
• Report to senior management on clinical issues and participate in audits.
• Assist with other duties as delegated by Management.

Education/Experience Requirements:

• QQI Level 5 Social, Healthcare degree required
• 2-3 years’ experience in home care, health care or senior-related industry required, an equivalent combination of education and work experience may be considered.
• Computer literacy skills is a must.
• You will be required to have a full clean driver's licence and access to your own car.

The Care Manager will be responsible to:

• Conduct consultations and prepare care plans.
• Conduct risk assessments in the home and mitigate any risks
• Monitor, mediate and manage both client and Caregiver concerns and issues.
• Carrying out Quality Assurance (QA) visits in client homes, developing and reviewing care plans for clients and conducting service enquiries and care consultations as needed.
• Through the QA visits, ensure client care plan is in line with their care needs, following up with family and healthcare professionals as appropriate.
• Ensuring the care being delivered in client’s homes is meeting HCCI, ISO & Franchise Standards, local policies and the requirements of the Health Service Executive (HSE)
• Caregiver Supervisions to be kept up to date.
• Mentoring and supporting a large team of Caregiver to carry out their roles effectively.
• Develop and maintain client relationships and client family/ caregiver relationships. - Build networks and enhance existing relationships with health care professionals and multi-disciplinary teams in hospitals and various community settings
• Report and consult with senior management on clinical issues as appropriate.
• To participate in clinical audit as identified by the organization.
• Assist with other duties as delegated by the Management Team.
