Social Care Facilitator
Salary: £31,586 - £36,124 per annum.
Hours: Full time, 37 hours per week.

We currently have new opportunities for Social Care Facilitators across the following teams:
Adult Disability Team
Based at Moat Lane in central Solihull, our highly experienced, passionate and dedicated Adult Disability team support adults with a learning disability, physical disability and/or autism as well as their carers.The team is responsible for assessing people’s needs, identifying appropriate sources of support, reviewing support in place and safeguarding vulnerable adults from abuse.
Community Older People’s Team
Based at Moat Lane, central Solihull, this busy and supportive team are responsible for supporting, safeguarding and assessing the needs of older adults over the age of 70, people with dementia and their carers.
Mental Health Team
Based at the Maple leaf centre, the Mental Health Team is an enthusiastic and diverse team who are passionate about changing the lives of people with care and support needs, and carers, through the promotion of wellbeing and choice, making a real difference and enabling them to increase their independence and have fulfilling lives, free from harm and abuse.
As a Social Care Facilitator in Solihull, you will work to a strengths-based model of assessment and support, promoting independence and well-being to achieve better outcomes for people, carers and their families.
To be successful in this role, you will be someone who can build a rapport quickly and able to prioritise and plan effectively and be creative with available resources.
You will join Solihull at an exciting time as the Council is transforming the way it provides its services.
