Job summary

A healthcare provider in London is seeking a District Nurse to support the Team Leader in caseload and staff management. Job description

The successful candidate will be required to support the DN Team Leader with leadership initiatives including caseload and staff management within the District Nurse Service structure. Depending on the demand of service, the post holder will be expected to support with any new initiatives including post Covid business as usual. You will also be required to complete home visits to provide nursing care, as well as providing complex patient management and support.

You will be expected to manage a clinical caseload which will meet the needs of all patients across the local landscape and network. The post will also allow unlimited opportunities to develop clinical skills and test your leadership styles accordingly. The role requires the post holder to work weekends on a rotational basis to support and coordinate weekend working across the locality.

Key Responsibilities
  • Supporting the DN Team Leader with leadership initiatives, caseload and staff management.
  • Supporting new initiatives and post Covid business as usual.
  • Completing home visits to provide nursing care and complex patient management.
  • Managing a clinical caseload across the local landscape and network.
  • Working weekends on a rotational basis to support and coordinate weekend working across the locality.
  • Using clinical judgement and risk assessments to keep patients safe.
  • Safeguarding patients and recognizing and responding to safety concerns.
  • Escalating safety concerns and acting as an effective advocate.
  • Being open and transparent about practice.
  • Supervising the work of others and maintaining accurate records.
  • Maintaining compliance with mandatory training requirements.
Clinical Outcomes
  • Being a confident, autonomous practitioner who takes responsibility for care and oversees the work of others in the team.
  • Providing skilled, evidence based nursing care that adheres to agreed policies and procedures.
  • Working with patients and families in all stages of the care planning process including assessing risks and needs.
  • Acting as a patient advocate in the multi-disciplinary team and ensuring the team responds to patient needs.
  • Communicating well with all members of the team and encouraging the team to reflect on practice.
  • Implementing sustainable initiatives that improve clinical outcomes through audits, benchmarking and quality improvement initiatives.
Patient Experience
  • Working in partnership with patients, families and carers and gaining consent to involve people in decision making.
  • Signposting patients and carers to recovery and wellbeing services.
  • Reassuring people by being professional, responsive, knowledgeable and confident.
  • Demonstrating clinical leadership and challenging poor behaviour to create a positive, inclusive culture.
  • Responding to complaints or concerns effectively and quickly in line with Trust policy.
Supporting yourself and others
  • Taking a lead role in gathering and recording evidence that CQC and local standards are being met.
  • Contributing to performance management and creating a positive culture for learning from mistakes.
  • Deputising for the team leader/ward manager and ensuring no fall in performance in their absence, including managing shifts.
  • Providing supervision and annual appraisals and giving constructive feedback to support individual development.
  • Participating constructively in own supervision and appraisal processes.
  • Using continuing professional development funds to develop a relevant clinical specialism, gain sign‑off mentorship skills or undertake accredited leadership or management modules.
