Job summary

Home Group Limited in Consett is seeking a Support Coordinator to lead support planning and coordinate to deliver person-centred assistance for young people experiencing homelessness. You will balance safeguarding, admin, and meetings while guiding colleagues and managing caseloads.

You will work with multi-agency teams to safeguard and empower customers, help them build independence, and prepare for move-on into the community.

Qualifications

• Experience in supported housing and with vulnerable children/young people.
• Level 3 Diploma in Care or willingness to work towards it.
• Experience assessing referrals and coordinating support.
• Ability to work on your own initiative and stay calm under pressure.
• Ofsted-registered service experience is desirable.

Responsibilities

• Lead support planning and coordinate to deliver person-centred support.
• Balance support, safeguarding, admin and meetings while guiding colleagues and managing caseloads.
• Support Customers with daily tasks like budgeting, cooking and attending appointments.
• Work with multi-agency teams to safeguard and empower our customers.
• Help customers build independence and prepare for their move-on into the community.
