Job summary

Dementia Oxfordshire, a service provided by Age UK Oxfordshire, is seeking a Dementia Adviser (Young Onset Dementia) to work hybrid from Abingdon. You will support people with dementia, their families and unpaid carers, providing information and planning access to services.

You will manage a caseload, visit clients at home, collaborate with health and social care professionals, and develop person-centred plans. DBS check is required; car access is essential.

Qualifications

• Excellent interpersonal and communication skills for engaging with clients, carers and professionals.
• Ability to manage a caseload and maintain accurate records.
• Experience working with volunteers and delivering information/awareness sessions.

Responsibilities

• Support people with a diagnosis, their families and unpaid carers by enabling them to plan ahead and express wishes for future welfare.
• Provide accurate, accessible information to help clients make informed choices.
• Develop person-centred support plans and work with health, social care and voluntary sector partners.
• Offer multiple support mechanisms including face-to-face, telephone, digital and group activities.
• Be a named contact for families and manage caseload with timely progress notes and reports.
• Attend team meetings and contribute to service improvement.
