Job summary

Alzheimer's Society in Cardiff seeks a part-time Community Support Worker (14 hours/week) to deliver one-to-one support to people affected by Young Onset Dementia across the Cardiff and Vale of Glamorgan area. The role involves home visits and community-based activities with a flexible approach to work from home as needed.

The successful candidate will have dementia experience, strong communication and organisational skills, and the ability to travel across the region to support clients in

Qualifications

• Understanding of dementia and needs of people living with dementia.
• Experience of working with people affected by dementia including Young Onset Dementia.
• Resilience to listen and empathise during challenging times.
• Strong communication and listening skills for personalised support.
• Strong organisational and time management skills.
• Excellent relationship-building skills within and outside the organisation.
• Ability to travel across Cardiff and the Vale of Glamorgan.
• Good IT skills to maintain records and update systems.

Responsibilities

• Provide one-to-one support in community settings and via home-based visits.
• Support individuals to build confidence, maintain independence and engage in activities.
• Assist with accessing public transport, appointments, and community activities.
• Record interactions accurately and maintain updated systems.
