Job summary

Age UK Oxfordshire is seeking a self-motivated Dementia Adviser for Young Onset Dementia (YODA) to provide specialist information, guidance and practical support to people with dementia, their families and unpaid carers across Oxfordshire. The post is primarily home-based with regular client visits and video/phone contact, requiring travel within Oxfordshire and line management by the Team Manager.

Qualifications

• Good general education.
• Training in dementia and/or dementia care.
• NVQ Level 3 or equivalent relevant to health, social care or community support.
• Experience supporting people in a caring role, preferably those living with dementia.
• Good listening and problem-solving skills with guided conversation approach.
• Experience of advice work.
• Experience of working with volunteers.
• Experience of facilitating groups.
• Experience of working with both statutory and voluntary organisations.

Responsibilities

• Support people with a diagnosis, their families and unpaid carers by enabling them to plan ahead, including financial and legal planning.
• Ensure they have accurate information to help make informed choices.
• Adopt a person-centred plan with clients and families, respecting choices and lifestyle.
• Collaborate with health, social care, employment and voluntary sector professionals.
• Develop and facilitate psychosocial interventions, groups and activities.
• Provide face-to-face, telephone, digital and group support.
• Understand a wide range of dementia diagnoses to tailor advice and support.
• Support families to understand dementia and access appropriate support.
• Be a named contact and manage caseload efficiently.
• Record and monitor all work, maintaining confidentiality and reporting progress.
