Nottinghamshire Healthcare NHS Foundation Trust is seeking a clinician to work within the Diagnostic service for the MAS Service and Young Onset Dementia Service, reviewing referrals for patients aged over 65 and under 65 respectively.

You will screen referrals for appropriateness, complete pre-assessment, identify reversible causes of memory problems, and conduct ongoing clinical risk assessments. A full UK driving license is required; DBS check is funded via salary deductions after appointment.

Qualifikationen

• DBS check required (costs deducted from salary).
• Full UK driving licence and access to a vehicle for business use.

Aufgaben

• Review and accept referrals for MAS Service and Young Onset Dementia Service.
• Screen referrals for appropriateness and complete pre-assessment.
• Identify reversible causes of memory problems and address them.
• Conduct ongoing robust clinical risk assessments.
• Act as point of contact for patients, carers, and referrers.
• Liaise with the MDT and contribute to outpatient clinics at short notice.
