Job summary

Lincolnshire Community Health Services NHS Trust is recruiting a Care and Wellbeing Navigator to support the day-to-day delivery of a specialist Tier 3 Weight Management Service. You will ensure a coordinated, positive patient experience from referral through to programme completion.

You will manage referrals, support patient engagement, coordinate meetings and communications, and maintain service data and records.

Qualifications

• Experience in an administrative, coordination or patient-facing role.
• Highly organised and able to manage competing priorities.
• Communicates confidently and professionally with a wide range of people.
• Strong IT skills, including Microsoft Office and database systems.
• Able to work independently while contributing to a wider team.
• Compassionate, person-centred approach.
• Interested in health improvement and behaviour change.
• Enjoys identifying opportunities to improve processes and services.

Responsibilities

• Support day-to-day delivery of a specialist Tier 3 Weight Management Service.
