Job summary

Derbyshire Community Health Services NHS Foundation Trust is recruiting a Weight Management Adviser for its Specialist Tier 3 Weight Management Service. You will join a multidisciplinary team supporting adults with complex obesity through personalised, evidence-based interventions and group education sessions, both face-to-face and virtually.

The role emphasises compassionate care, lifestyle change coaching, and matching care to patient needs, with opportunities to contribute to service

Responsibilities

• Deliver evidence-based weight management assessments, education, and ongoing support to adults with complex obesity.
• Manage a caseload of patients receiving Tier 3 weight management, providing personalised coaching and behaviour change interventions.
• Collaborate with the multidisciplinary team to ensure access to dietetic, psychological and medical support when needed.
• Facilitate and support both face-to-face and virtual group education sessions.
• Help patients set realistic goals, monitor progress, and identify barriers to sustained lifestyle change.
• Record and interpret outcome measures in line with service requirements and quality standards.
• Undertake baseline assessments of physical health and lifestyle factors in accordance with service protocols.
• Contribute to service development, quality improvement, and creation of patient resources.
• Maintain accurate clinical records and work in line with governance standards.
