Job summary

Gloucestershire Hospitals NHS Foundation Trust is seeking a motivated Band 3 Vascular Pathway Tracker (30 hours per week) to support patients through their vascular treatment journey. The role involves monitoring pathways, maintaining records, and assisting MDT meetings with a patient-centred approach.

The post requires excellent organisational and communication skills, high attention to detail, and the ability to manage competing priorities in a busy NHS environment.

Qualifications

• GCSE maths and English at grade C or above is required.
• Experience using Microsoft Office (Outlook, Word, Excel, PowerPoint) in a busy setting is essential.
• Experience in hospital-based administration is advantageous.

Responsibilities

• Track and monitor patients throughout their vascular pathway, ensuring records are up to date and within timescales.
• Work with the Vascular Pathway Navigator to progress patients on pathways and meet targets.
• Update patient information on Trust systems, including tracking lists and worklists, ensuring data accuracy.
• Escalate any delays or issues affecting patient care at pathway level.
• Monitor progress and communicate updates to the Pathway Navigator and colleagues; escalate risks as needed.
• Support the MDT process by preparing patient information and recording outcomes.
• Liaise with clinical, administrative and operational teams to facilitate pathway progression.
• Use initiative to resolve routine pathway issues under pressure.
