Job summary

Colchester Hospital University NHS Foundation Trust is seeking an experienced Nurse Practitioner (Nurse Prescriber) to join the Urgent Community Response Service (UCRS) in East Suffolk and North Essex.

The role involves delivering urgent care in patients' homes and care home settings, assessing, diagnosing, prescribing and managing a wide range of urgent healthcare needs with autonomy and clinical leadership.

Qualifications

• Registered Nurse Prescriber qualification required.
• Experience in urgent or community care is desirable.

Responsibilities

• Undertake rapid assessments and manage urgent clinical presentations in community/home settings.
• Prescribe appropriate treatments and interventions within GP/UCRS protocols.
• Deliver short-term, intensive support to prevent hospital admission.
• Provide expert clinical advice to patients, carers and healthcare professionals.
• Work with multi-disciplinary teams and primary care colleagues to coordinate care.
