The role focuses on delivering rapid in-home assessments and support to prevent unnecessary hospital admissions, working across Dorset’s community settings.

You will operate as part of the UCR team with supervision from senior staff, contributing to timely patient discharges and promotion of independence and wellbeing in the home

Responsibilities

• Provide a 2-hour response to patients in their own home or care home, to assess and support with health needs.
• Work as part of the Urgent Community Response multidisciplinary team under supervision of qualified staff.
