Job summary

Northamptonshire Healthcare Foundation Trust is seeking a Community Staff Nurse to join the Urgent Community Response team. Work is in patients' homes with IV therapy, aiming to avoid hospital admissions and enable quicker discharge.

Qualifications

• Registered Nurse with current NMC registration.
• Evidence of continuing professional development.
• Experience assessing patients and working autonomously.
• Valid UK driving licence with access to a vehicle on duty.

Responsibilities

• Assess patients and families to identify healthcare needs.
• Deliver treatments including IV therapy in patients' homes.
• Monitor care quality and take professional accountability.
• Set goals with therapists and doctors to support recovery.
• Manage sub-acute patients to shorten hospital stays.
• Respond to needs at short notice, including weekends.
• Update patient records using technology.
