Job summary

Central and North West London NHS Foundation Trust (CNWL) is recruiting a Band 6 UCR Practitioner for the Home 1st Urgent Community Response team in Milton Keynes on a fixed-term basis for 10 months.

You will provide rapid assessment and intervention for patients in their own homes, helping to avoid hospital admissions and support timely discharge. The role requires autonomous clinical decision-making within a multidisciplinary team and across urgent community pathways.

Qualifications

• Registered Nurse with current NMC registration.
• HCPC Registration.
• Evidence of other professional training, CPD and PDP.
• Advanced Assessment Skills or willingness to work towards.

Responsibilities

• Provide advanced clinical assessment, diagnosis and treatment for patients with acute health crisis at home or care home.
• Triage referrals from GPs, Consultants, NHS 111, care homes and community services.
• Respond to urgent referrals within a two-hour timeframe with patient-centred care.
• Work autonomously and with multidisciplinary teams to determine care pathways.
• Undertake risk assessments, clinical decision-making and care planning.
• Support Virtual Ward, intermediate care and reablement pathways.
• Deliver urgent assessment and intervention for falls and acute illness.
• Maintain accurate clinical documentation and professional standards.
