Job summary

Provide CIC is recruiting a Nursing Associate/Practitioner to join its Urgent Community Response Team in Chelmsford. You will deliver high-quality, patient-centred care in people’s homes, helping avoid hospital admissions and supporting independence.

The role involves comprehensive home assessments, venepuncture, catheter care and managing acute episodes, with training provided and opportunities to work autonomously within a multidisciplinary team.

Qualifications

• Qualified Nursing Associate/Practitioner with community care experience.
• Ability to work independently and as part of a multidisciplinary team.
• Experience in venepuncture and catheter care preferred.

Responsibilities

• Carry out comprehensive assessments in patients' own homes.
• Implement and evaluate individual care plans under supervision.
• Maintain clear communication with patients, relatives, and team.
• Deliver acute care including venepuncture and catheter care.
• Support UCRT team to ensure holistic home-based care.
