Job summary

Hampshire and Isle of Wight Healthcare NHS Foundation Trust is seeking an Urgent Community Response Nurse (UCR) to deliver advanced community care within the Southeast Division. You will assess health needs, perform clinical reviews, and manage patients in their homes via our virtual ward, contributing to reduced hospital admissions.

Experience in community care is desirable, with full training provided and ongoing post-qualification courses including diagnostics and independent nurse

Qualifications

• BSc Nursing or RN registered with NMC.
• Practice assessor/supervisor and post-registration specialist training.
• Access Course / Diploma in Health Care Studies; driving licence is required.

Responsibilities

• Manage a defined community caseload via integrated care team.
• Coordinate and delegate work ensuring timely referrals and care plans.
• Aim to prevent hospital admission and support timely discharge to the community.

Education
