Job summary

South Central Ambulance Service NHS Foundation Trust is seeking an Urgent Community Response Nurse / Paramedic Practitioner to provide high-quality, rapid clinical care in patients' homes and community settings. You will perform advanced assessments, make autonomous clinical decisions, and work closely with health and social care colleagues to prevent hospital admissions and support early discharge.

Qualifications

• Registered nurse or paramedic with current professional registration.
• Experience in urgent community care and home-based assessments is desirable.
• Ability to work autonomously and make clinical decisions.

Responsibilities

• Undertake comprehensive clinical assessment, diagnosis and treatment planning for patients referred to the Urgent Community Response service.
• Triage referrals from GPs, Consultants, NHS 111, and other stakeholders to ensure appropriate intervention.
• Respond to urgent referrals within a two-hour timeframe in line with national UCR standards.
• Deliver advanced clinical care and short-term interventions to prevent unnecessary hospital admissions.
• Support patients in their homes, care homes and community settings with monitoring and care planning.
• Work with multidisciplinary teams to coordinate holistic care and ensure patient safety.
