Job summary

Target Housing is seeking two experienced Housing Support Workers to join the THRIVE Project in Sheffield. You will provide tailored one-to-one and group support to individuals with complex needs, using a structured case management approach to promote tenancy sustainment and independent living.

As part of a psychologically informed and trauma‑informed team, you will conduct risk assessments, develop support and safety plans, and work with partners to empower service users while maintaining

Qualifications

  • Experience supporting individuals with multiple complex needs.
  • Ability to engage hard-to-reach clients.
  • Strong relationship-building and communication skills.
  • Experience with risk assessment and support planning.
  • Knowledge of PIE and TIP.
  • IT literacy for recording and reporting.
  • Full UK driving licence and access to a roadworthy vehicle.

Responsibilities

  • Deliver 1:1 and group support using a case management approach.
  • Complete comprehensive risk assessments, support plans, and safety plans.
  • Motivate and empower clients to achieve personal goals.
  • Design and deliver activity programmes focused on independence and wellbeing.
  • Maintain accurate records in line with data protection standards.
  • Monitor tenancy compliance and address issues; build partnerships with external agencies.
  • Use motivational interviewing and reflective practice during case reviews.
