Job summary

Target Housing is seeking two experienced Housing Support Workers to join the THRIVE Project in Sheffield. You will provide tailored one-to-one and group support to individuals with complex needs, using a structured case management approach to promote tenancy sustainment and independent living.

As part of a psychologically informed and trauma‑informed team, you will conduct risk assessments, develop support and safety plans, and work with partners to empower service users while maintaining

Qualifications

Experience supporting individuals with multiple complex needs.
Ability to engage hard-to-reach clients.
Strong relationship-building and communication skills.
Experience with risk assessment and support planning.
Knowledge of PIE and TIP.
IT literacy for recording and reporting.
Full UK driving licence and access to a roadworthy vehicle.

Responsibilities

Deliver 1:1 and group support using a case management approach.
Complete comprehensive risk assessments, support plans, and safety plans.
Motivate and empower clients to achieve personal goals.
Design and deliver activity programmes focused on independence and wellbeing.
Maintain accurate records in line with data protection standards.
Monitor tenancy compliance and address issues; build partnerships with external agencies.
Use motivational interviewing and reflective practice during case reviews.
