Job summary

• Experience delivering housing and support services to vulnerable residents.
• Experience creating support plans and risk assessments.
• Experience collaborating with multi‑agency partners.

Responsibilities

• Assist residents moving in/out of Target accommodation and carry out risk assessments.
• Develop and implement personalised support plans with realistic goals.
• Act as residents' keyworker, offering advice and support for independent living.
• Assist residents in budgeting, managing income and benefits.
• Help residents understand rights and responsibilities under occupancy agreements.
• Monitor and review progress, updating plans as needs change.
• Ensure rent payments are up to date and support benefit claims where needed.
• Liaise with external bodies to coordinate support and resources.
