Responsibilities

• Provide personalised support to individuals with multiple needs to enable housing stability.
• Conduct 1:1 sessions daily and participate in group work.
• Coordinate with health, housing and social care partners to deliver integrated services.
• Assist service users to access education, employment and independent living options.
