Job summary

Croydon Council is seeking a Social Worker for the Transitions Service (18-25) to support young adults with disabilities as they move into adulthood. You will work collaboratively with individuals, families and partners to promote independence, wellbeing and positive outcomes.

You will undertake strengths-based assessments under the Care Act 2014, develop outcome-focused support plans, and provide information on community resources while safeguarding and managing risks.

Qualifications

• Qualified social worker with current registration.
• Experience with Care Act 2014 and safeguarding.
• Strong assessment and risk management skills.

Responsibilities

• Undertake proportionate, strengths-based assessments under Care Act 2014.
• Co-produce outcome-focused support plans with multi-agency partners.
• Provide information and guidance on community resources.
