Job summary

Lancashire County Council’s Adult Social Care service in East Lancashire is seeking an experienced Transitions Co-Ordinator to support young adults as they prepare for adulthood and move into adult services, helping to ensure the right support is in place at the right time.

We offer a strengths-based approach, collaborative working with families and partners, extensive supervision, and opportunities for professional development within the Young Adults Service.

Qualifications

• Social Work registration with SWE is required or application in progress.
• Must be able to travel and use a car for duties.

Responsibilities

• Support young people as they prepare for adulthood and transition to adult services.
• Collaborate with families, partners and carers to ensure coordinated, person-centred support.
