Job summary

Bolton Council is recruiting a qualified social worker to join the Transition team, supporting young people aged 14–25 with special education needs and disabilities. You will collaborate with Children and Adult services to plan and implement smooth transitions to adulthood and assess carers as needed.

The role requires driving licence, DBS clearance, and a CQSW/DIPSW/SW degree or equivalent. The post is permanent, 29.6 hours per week, with a competitive salary and extensive employee benefits.

Qualifications

• Demonstrate ability to work with service users to ensure informed decisions within statutory frameworks/local policies.
• Ability to identify and assess needs, options and urgency to plan and act accordingly.
• Identify, assess and manage risk to service users while balancing rights and responsibilities.

Responsibilities

• Undertake holistic assessments and develop transition plans for young people 14–25 with special educational needs and disabilities.
• Discharge duties in accordance with the Care Act 2014.
• Identify and assess informal carers' needs and provide appropriate support.
• Collaborate with Children and Adult services and partners to ensure smooth transitions to adulthood.
