Job summary

Wiltshire Council is seeking an Experienced Social Worker to join the Moving on Service in the Wiltshire area. This role focuses on delivering person‑centred support to young people with learning disabilities and autism as they transition into adulthood.

You will work in a collaborative, supportive team, handling a varied caseload and using enablement approaches while liaising with health, education and other partners.

Qualifications

• Two years’ post-qualification experience.
• DipSW or equivalent qualification.
• Social Work England registration required.

Responsibilities

• Manage a varied caseload and complete Care Act Needs Assessments using strength-based, person-centred approaches.
• Work across children’s and adult services to support young people’s transition to adulthood.
