Job summary

Bath and North East Somerset Council in Keynsham is seeking a Transitions Social Worker to support young people aged 16 to 25 as they transition into adult care. You will carry out first adult care assessments and develop care and support plans in collaboration with multi-disciplinary teams.

Qualifications

• Qualified Social Worker with social work degree or equivalent.
• Experience with wider range of young people and adults including Autism, mental health conditions or learning disabilities is advantageous.
• Registered with Social Work England.
• Must have a Full UK Driver’s Licence and access to a car.

Responsibilities

• Undertake first adult care assessments and reviews.
• Create care and support plans meeting needs of service users and carers.
• Conduct Mental Capacity assessments and Best Interests Decisions when appropriate.
• Perform risk assessments and engage MDT in positive risk taking strategies.
• Complete S117 Aftercare reviews and Continuing Health Care Checklists as needed.
